- The following table compares the three most commonly studied cerebrolysin dosing protocols for acute ischemic stroke based on published trial data.
- CASTA Standard
- 30mL IV daily × 10 days
- Within 12 hours of onset
- NIHSS at 90 days
- 18% greater reduction in neurological deficit
- Gold standard for acute intervention. Strongest evidence base, well-tolerated, requires hospitalized administration
- Extended Maintenance
- 30mL IV daily × 10 days, then 10mL 3×/week × 3 weeks
- Within 24 hours of onset
- Modified Rankin Scale at 6 months
- 22% higher rate of functional independence (mRS 0–2)
- Best for maximizing long-term recovery. Logistically complex, requires outpatient infusion access
- Early High-Dose
- 50mL IV daily × 10 days
- Within 6 hours of onset
- Barthel Index at 90 days
- No significant improvement over 30mL dose
- Not recommended. Increased infusion reactions without additional efficacy, cost-prohibitive
